Job summary
Rochford Primary Care Network has a fantastic opportunity for a healthcare assistant - population health, to join the practice to support the PCN team, on a full-time basis.
Working under the direct supervision of the population health practitioner, the post-holder will work within their own competencies and will be expected to conduct clinical aspects of NHS Health Checks.
Main duties of the job
1. Conduct NHS health checks, new patient health checks & over 40 health checks.
2. Conduct baseline observations such as pulse oximetry, blood pressure etc recording findings accurately.
3. Conduct Body Mass Index (BMI) checks.
4. Helping raise awareness of health & well-being & how it can be promoted, providing advice to patients.
5. Supporting patients to adopt health promotion strategies and encourage principles of self-care.
6. Deliver opportunistic health promotion & lifestyle advice.
7. Supporting the practitioner with the management of chronic diseases in a community-based setting.
8. Blood pressure and hypertension recording and assessment.
9. Dementia screening and signposting.
10. Give accurate & appropriate information to patients and groups within own competencies.
11. Work to address health disparities within the community by advocating for equitable access to healthcare & public health resources.
12. Assist in planning & executing health promotion campaigns & initiatives, including health screenings & awareness events.

Essential
23. GCSE A-D or grade 3-9 in Maths and English or equivalent.
24. Level 3 or equivalent Regulated Qualification Framework (RQF) or National Vocational Qualification (NVQ) level 3 in Health and Social Care.
25. Care Certificate

26. Vaccination and Immunisation Foundation Course for both adults and children
27. Phlebotomy Certification
28. Ear Syringing Certification
Experience
Essential
29. Experience of working in a healthcare setting.
30. New patient medicals including height, weight, BP, pulse, and wellbeing advise.
31. Ability to record accurate clinical notes.
32. Basic observation taking BP/Pulse/SATs/BM
33. Competent in Clinical Waste and Sharps policy and procedure.
34. Excellent communication skills (written and oral).
35. Strong IT skills.
36. Competent in the use of patient record management systems: SystmOne.
37. Ability to work as a team member and autonomously.
