We are looking for a self‑motivated and passionate Qualified Practitioner registered as either a Mental Health Nurse with the NMC, an Occupational Therapist with the HPCSE, or a Qualified Social Worker registered with Social Work England to join our well established Early Intervention in Psychosis Service in Mid Essex.
Position: Senior Care Co‑ordinator, Mid Essex First Episode Psychosis Team, based at The Gables in Braintree.
Role: Work across Mid Essex providing multi‑disciplinary care, promoting recovery and social inclusion for individuals aged 14–65 with first‑episode psychosis.
Main Duties
* Fully conversant with and uphold professional code of conduct.
* Ensure care delivered in accordance with Trust’s care programme approach guidelines.
* Participate in initial assessment to determine appropriateness for the FEP Team and liaise with referring agencies to identify alternative agencies as appropriate.
* Undertake risk assessments with clients and plan effectively within care plan and MDT.
* Ensure interventions undertaken within established time scales and in line with best evidence‑based practice and operational policy.
* Work flexibly, participating in demands of the FEP Service; pattern subject to change to meet client needs and National Service Framework guidelines.
* Provide clinical supervision and professional leadership to junior staff and promote evidence‑based practice.
* Attend mandatory training and keep up to date.
* Keep updated on legislative government initiatives relevant to Early Intervention in Psychosis.
* Undertake care coordination of a caseload of clients.
* Carry responsibility for assessment of care needs, development, implementation, and evaluation of recovery‑based care plans.
* Participate in data collection and analysis—entry/exit pathways, duration of untreated psychosis, outcome measures (Dialog & QPR).
* Maintain effective individualised care through assessment of user and carer's problems; plan, implement, evaluate interventions and produce clear, accurate documentation.
* Undertake initial health and social needs assessments and participate in MDT meetings for client selection, allocation to CPA and regular reviews.
* Plan creatively to meet client needs, providing evidence‑based interventions.
* Prioritise and manage workload to ensure effective management of a defined group of clients with complex needs.
* Work with colleagues to improve quality of services.
* Participate in annual audits relevant to the clinical pathway.
* Participate in the First Episode Psychosis Duty Rota.

Person Specification
Essential
* Years of experience.
* Evidence of delivery of psychosocial interventions in clinical practice.

* Appropriate professional registration.
