Job Summary

We are excited to announce a fixed-term vacancy for a Community Care Officer in the Ashfield area. This role is perfect for motivated and enthusiastic individuals who want to help others live their best lives in the comfort of their own homes. You will work closely with individuals to understand their goals and aspirations, using strength-based approaches to promote choice, control, and personalisation in their care.


Role Responsibilities

* Assessing Needs: Identify and review the needs of individuals and their carers, coordinating solutions to meet personalised outcomes.
* Promoting Independence: Encourage individuals to manage their own needs and risks, fostering a sense of autonomy.
* Connecting Resources: Link individuals to community resources and support networks, ensuring they receive the right help.
* Monitoring Care Packages: Commission and oversee care services within set timescales to ensure quality support.

Collaborating with Professionals: Work alongside social workers and health colleagues to provide holistic assessments and support.


Required Skills And Qualifications

* A relevant health or social care qualification (NVQ Level 3 or equivalent).
* At least two years of experience in a health or social care setting.
* Knowledge of community care legislation, including The Care Act 2014 and the Mental Capacity Act 2005.
* Proficiency in using IT applications for record-keeping and reporting.
