Overview
The Pall Mall Surgery has a fantastic opportunity for an experienced advanced nurse practitioner to join our team on a part-time basis, for 30 hours per week.
Responsibilities
* Work autonomously in dedicated ANP clinics, providing direct patient access via face-to-face, telephone, or video consultations.
* Act as a first point of contact for patients presenting with undifferentiated, undiagnosed problems, using advanced skills in history-taking, physical examination, problem-solving, and clinical decision-making to establish a diagnosis and management plan.
* Be accountable for their own clinical actions and practice safely within their professional scope, using expert knowledge and advanced skills to deliver holistic care in a primary care setting.
* Request and interpret appropriate investigations, including invasive and non-invasive diagnostic tests.
* As an independent prescriber, prescribe safely and effectively in line with best practice, national guidance, and the legislative framework, ensuring cost-effective use of medicines.
* Provide safe, evidence-based, and individualised care to patients and their families, developing ongoing management or support plans with an emphasis on prevention and supported self-care.
* Promote health and wellbeing through proactive health education, screening, lifestyle advice, and therapeutic communication skills.
* Refer patients appropriately and directly to secondary care or community services where required.
* Work with colleagues to ensure national and local clinical frameworks (e.g. diabetes, cardiovascular disease, respiratory health, mental health) are delivered.
* Contribute to quality improvement, including QOF, enhanced services, and local incentive schemes, supporting the practice to achieve and sustain high standards of care.
* Work collaboratively with the multi-disciplinary team to provide integrated and seamless patient care.
* Identify and respond to community health needs, supporting service development and innovation within the practice.
* Develop a clinical area of special interest to improve outcomes for the practice population.
* Audit practice outcomes against standards, using findings to drive learning and improvement.
* Actively implement safeguarding protocols for children and vulnerable adults, escalating concerns promptly.
* Contribute to commissioning and service development initiatives aimed at improving patient care.
* Use IT systems effectively to support patient care, communication, and education.
* Maintain professionalism at all times, adhering to practice protocols, policies, and procedures.
Qualifications
* Independent prescriber qualification.
* Proven experience in autonomous assessment, diagnosis, and management of patients with acute and long-term conditions.
* Strong clinical knowledge, decision-making, and communication skills.
* Sound understanding of NICE guidelines and local clinical pathways.
* Experience in mentoring, leadership, and clinical audit.
About the Practice
The Pall Mall Surgery is a large, modern GMS training practice operating from a contemporary health centre in Leigh On Sea. The practice serves 21,500 patients from birth to end-of-life care and holds a CQC rating of Good in all areas. We have a dedicated and supportive work environment with experienced clinicians and a strong administration team. We operate a total triage system, offer 15-minute face-to-face appointments and 10-minute telephone consultations, and welcome ideas from our team to maintain high standards of care.
