Make a real difference to children, young people and families.
We’re looking for committed practitioners to deliver high‑quality, home‑based Early Help support (Level 3 need) using a whole‑family approach.
You’ll work with children aged 0–25* and their families to strengthen resilience, improve outcomes and prevent escalation to statutory services.
*Support for ages 19–25 applies only where the SEND Code of Practice is relevant.
What you’ll do
* Take lead professional responsibility for families stepping down from Children’s Social Care—completing Family Help Assessments, coordinating TAF meetings and driving positive change.
* Provide ongoing support where needs increase and cases step up to Level 4, ensuring a smooth, consistent transition to statutory services.
* Deliver intensive, targeted support to vulnerable children and young people, including those with complex emotional or mental health needs.
* Create and coordinate Team Around the Family plans, ensuring timely review and effective multi‑agency working.
* Use de‑escalation and strength‑based approaches to reduce risk, build on family strengths and improve stability.
* Support parents/carers with practical, positive parenting strategies—including facilitated family meetings.
* Maintain accurate, high‑quality case recordings, assessments, chronologies and plans.
* Visit children and families at home to provide tailored support and ensure children’s needs are met.
* Work in line with Family Help Practice Standards.
* Champion safeguarding and ensure all actions meet statutory duties and guidance.
