Overview
We are recruiting to a fixed-term Band 6 role within the Lincoln Community Mental Health Team. The successful applicant will be a motivated and enthusiastic RMN joining our community team in Lincoln to deliver high quality, person-centred mental health care.
Responsibilities
* Provide high quality, person-centred care to patients, families and carers.
* Act as care co-ordinator, undertake comprehensive evidence-based assessments and implement recovery-focused plans.
* Demonstrate knowledge of psycho-social interventions and support service users to work towards recovery goals, build self-resilience and promote independent living.
* Work within a friendly multi-disciplinary team (Community Mental Health Nurses, Recovery Practitioners, Occupational Therapists, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists, Admin staff).
* Collaborate with Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP practices and other partners to achieve optimal outcomes for patients and carers.
* Receive robust clinical and managerial supervision regularly and participate in ongoing professional development and evidence-based practice updates.
* Travel independently in the community with a hybrid approach to work when required.
Qualifications & Experience
* Registered Mental Nurse (RMN) with appropriate professional registration.
* Ability to undertake comprehensive assessments and work effectively within a multi-disciplinary team.
* Commitment to ongoing professional development and keeping up-to-date with current evidence-based practice.
* Ability to travel independently in the community and operate with a hybrid working pattern.
