Hospital Discharge Team Coordinator

We have an opportunity to work for the Single Point of Access Team as a Band 4 Hospital Discharge Co-ordinator.

More about the role

We seek a motivated team member to support patients in living independently at home post-hospitalization. Based at North Devon District Hospital, you will handle referrals, liaise with patients, relatives, and multidisciplinary teams, and manage data effectively. You should be proactive, problem-solve, and work well in a team.

* Provide business support, administrative leadership, and office management to ensure high-quality coordination services.
* Manage your workload, including financial and personnel administration, and supervise lower-band staff.
* Coordinate the Single Point of Access across North Devon to support crisis management, prevent unnecessary hospital admissions, and facilitate hospital discharges.
* Process Pathway 1, 2, 3 referrals within response times and handle admission avoidance referrals via phone, signposting appropriately.
* Input, store, analyze data, and collaborate on system improvements.
* Liaise with all involved teams and agencies, including GPs, social care, and hospital discharge teams.
* Ensure accurate rostering and cover staff shortages proactively.
* Support Response and Recovery Workers, including lone working and out-of-hours support, in line with trust policies.
* Maintain positive relationships with colleagues and external partners.
* Use IT systems like EPIC and CareFirst 6 for case management, ensuring information accuracy and communication.
* Signpost less urgent referrals to appropriate services, handle diverse information management tasks, and meet service targets.
* Maintain accurate electronic and written records and work flexibly across the team.

Working Pattern: 37.5 hours/week, 8:00-16:00, Monday-Friday.
