This is an excitingopportunity for Registered Nurses to work, learn, and qualify as a GeneralPractice Nurse in North East London (City and Hackney, Tower Hamlets, Newham,Waltham Forest, Redbridge, Barking & Dagenham and Havering).
Main duties of the job
You will be employedby a local GP practice/group of practices/PCN where you will gain experience ofworking in a clinical setting within Primary Care, whilst undertaking a PostgraduateDiploma in General Practice Nursing.
Every trainee willhave an experienced GPN as their mentor/supervisor during their training, alongsidereceiving support from the GPN VTS Scheme Co-ordinators. This support willguide you through a comprehensive competency-based approach to clinical care ina general practice setting.
Trainee posts will be fixed-termfor 12 months whilst you undertake the academic programme. You will be paid ona full-time salary (37.5 hours per week) on a training salary. All course fees willbe fully funded by NHS England (London).

About us
TheGPN VTS trainee will be able to specify their preferred borough/place withinNorth East London to work in and every effort will be made to adhere to thispreference, subject to clinical placements in employing practices beingavailable.

Job responsibilities
Todevelop the knowledge, clinical skills and attitude to provide high qualitycare and services based on the needs of the practice population. Supported bysenior nurses within the practice, you will deliver care within the boundariesof your role, focusing upon supporting patients to be healthy, monitoring oflong-term conditions, health promotion and screening activities. You will workcollaboratively with the general practice team to meet the needs of patients,supporting the development and implementation of policies and procedures.
Examples of Duties andResponsibilities that will be developed during the 12 month placement on the VTScheme:
Clinical
1.Supportpatients to adopt health promotion strategies that encourage patients to livehealthily, and apply principles of self-care;
2.Deliveropportunistic health promotion using opportunities such as new-patient medicalsand NHS Health Checks;
3.Implementand participate in national vaccination and immunisation programmes for bothadults and children. Provide travel advice and vaccinate appropriately oncetrained.
4.Assess,plan and support patients to self-manage long term conditions, running specificclinic sessions and maintaining robust recall systems;
5.Assessand care for patients presenting with uncomplicated wounds, including removalof sutures and clips;
6.Supportand manage health needs of women presenting for cervical cytologyconsultations and perform cervical cytology screening once trained;
7.Supportand advise patients requesting information relating to reproductive and sexualhealth;
8.Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, with support and guidance from senior membersof the team;
9.Assistsenior practitioners in providing minor-surgery sessions where this service isprovided;
10.Recognise,assess, and refer patients presenting with mental health needs in line withlocal policy and practitioner competence.
11.Recogniseand highlight where clinical skills are outside of your clinical remit andexpertise.
12.Assist inand perform, Urinalysis, Peak Flow Readings, Venepuncture, Blood Pressuremeasurements fitting of and recording 24-hour BP readings (depending onlocation) and other specific tests as appropriate for the individual practicerequirements. Adhering to appropriate recall systems.
Communication
1.Communicateeffectively within a team in general practice. Working alongside various healthcare professionals with a common goal to support, manage and treat patients.
2.Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.
3.Utilisecommunication skills to support patients to adhere to prescribed treatmentregimens.
4.Shareall training materials, education and CPD certificates with the employingpractice.
5.Anticipatebarriers to communication and take action to improve communication.
6.Actas an advocate when representing the patients and colleagues viewpoints toothers.
Administration
1.Developan understanding the wider primary care system including but not limited toPCNs, ICBs, ICSs and neighbourhoods.
2.Tomaintain accurate patient records and enter onto the computer using agreed SNOMEDcodes;
3.Ensureaccurate completion of all documentation associated with patient care.
4.Tocollect data and participate in regular practice audit and research projects;
5.Tocontribute to any practice development plans.
General
1.Tomaintain own annual registration with the NMC and professional indemnity andadhere to The Code: Professional standards of practiceand behaviour for nurses, midwives and nursing associates;
2.Tomaintain a safe and therapeutic environment for all the Primary Health Care Teamstaff, patients, carers and colleagues from allied organisations;
3.Ensureall statutory regulations (e.g. Health & Safety, COSHH, etc.) areimplemented and adhered to;
4.Toalways promote a professional approach atall times by attire and attitude to patients and staff both in and outof the practice;
5.Tomaintain own Continuous Professional Development (CPD) and ensure relevant CPD attended.In addition, CPD must be kept up-to-date through annual appraisal in line withthe requirements of the Practice and The NMC Code (2018);
6.Attendand participate in staff, practice, place-based meetings and in-house clinicaltraining sessions.
7.Demonstratean understanding of the breath and scope of the role as a general practicenurse working in primary care;
8.Workingwithin limitations, being aware of own boundaries.

* Educated to degree level (A) BSc Hons 2:2 (minimum)
