Job overview
We are looking to recruit a highly skilled individual to join and lead our mental health (MH) flow team.
The MH flow team support the MH crisis pathways in supporting patients to be discharged to their onward destination safely and in a timely way.
This involves liaison with ward multidisciplinary team (MDT) and community team MDT.
The role involves high levels of liaison with multiple internal and external teams in our neighbouring boroughs and across the country where we may have stranded patients.
The role also supports the procurement and quality assurance of discharge to assess provision, bed and breakfast and enhanced care packages, provisions that also support flow through the MH crisis pathways.
Main duties of the job
1. Managing and leading the MH flow team.
2. Leading collaboration to support flow across the acute crisis pathway teams and the community teams.
3. Supporting MDT in decision making regarding discharges.
4. Establishing strong relationships with partners to support MH flow.

High Cost Area Supplement
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 pro rata for part time).

5. To manage the flow team
6. To support the implementation of relational care across the crisis pathway through the flow processes
7. To build effective partnerships with crisis pathway teams and community teams
8. To support MDT development in their understanding of ASC and housing issues that can impact on Flow
9. To build effective working relationships with Place leadership teams
10. To build effective working relationships with adult social care (ASC) and housing in our 4 ONEL places
11. To seek and collate information about ASC and housing in places outside of ONEL to support Flow
12. To work with a range private providers of supported accommodation, residential and nursing provision
13. To work with acute hospitals where there are complex cases
14. To set and review objectives, in the context of strategic developments within the services.
15. To work in partnership with patients and carers working towards the highest standards.
16. To establish effective means of service co-ordination to ensure care is delivered to the principles set out by the local Place for transformation.
17. To hold budgetary responsibility and accountability relating to all service areas within the remit of the post.
18. To effectively report against targets and provide insights into the performance of the services
19. To develop effective patient centred decision-making processes for any proposed changes
