Responsibilities
* Undertake strengths based social care assessments, care and support planning, and reviews in line with statutory responsibilities
* Manage a caseload that promotes choice, independence and wellbeing
* Contribute to safeguarding enquiries and best interest decision making
* Work collaboratively with health, housing, voluntary and community partners
* Maintain accurate and timely records using electronic case management systemsYou will be supported through regular supervision, reflective practice and ongoing learning, enabling you to develop your skills with confidence.
Qualifications
* Is a qualified Social Worker registered with Social Work England
* Has recent and relevant experience within adult social care (desirable within learning disabilities)
* Has sound knowledge of key legislation, including the Care Act, Mental Capacity Act and safeguarding frameworks
* Demonstrates strong analytical and decision making skills within appropriate timescales
* Is committed to person centred, strengths based practice and works with sensitivity, professionalism and compassion
