The successful candidate will have experience in transformation and collaboration which they will use to lead the development of Integrated Neighbourhood Teams in Buckinghamshire to deliver a positive impact for our communities.
This pivotal role will involve leading the integration of services across health and social care providers, with community and voluntary sector partners. With a shared ambition to deliver joined up care and support for people to live healthier lives.
We are looking for someone with a successful record of delivering transformation in a complex environment across multiple organisations or teams. The person will be solution focused with exceptional interpersonal and relationship building skills.

Main duties of the job
Neighbourhood teams are built around populations and geographies, reflecting and responding to how people live their lives and are intended to:
•streamline access to care and advice to meet the needs of infrequent users of health care services
•provide more proactive, personalised and multi-disciplinary care for people with more complex needs
•help people to stay well for longer, through a joined-up approach to
prevention.

Essential criteria
1. Evidence of continuing profession development

2. Masters degree or equivalent qualification / knowledge & experience relevant to role
Experience
Essential criteria
3. Experience of delivering complex change management projects/programmes, and/or service redesign across the whole the health and social care environment, or other comparable environments
4. Demonstrable track record of programme delivery within required timeframe and budgets across whole organisations and systems
Desirable criteria
5. Experience of matrix working across multiple teams and organisations and building effective partnerships in complex environments

6. Excellent interpersonal skills, with the ability to build strong working relationships and be able to motivate people, at all levels of the organisation
7. Ability to interpret highly complex information, and make decisions based on highly complicated and sometimes incomplete information
