Overview

An exciting opportunity for a Band 6 Junior Sister, Community Nurse, has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury – a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies, including Adult Social Care and a GP practice, so we can work together in a fully integrated way.


Responsibilities

The successful applicant will join a dynamic, friendly team of community nurses, community matrons, therapists and social care staff. You will provide nursing services to patients in the community and have the chance to work autonomously with the support of the wider multidisciplinary team within the hospital and across the West Suffolk Alliance.

You will:

* Provide clinical and supervisory leadership to staff and students within the team;
* Support the management of nursing staff by taking responsibility, alongside the District Nurse, for the day‑to‑day coordinating, planning and delivery of nursing services within the Community Healthcare Team;
* Manage patients with multiple and complex needs using evidence‑based practice to assess, plan, implement and evaluate interventions to improve patient health;
* Deliver nursing care on site at the health centre and in specialist clinics, using a wide range of nursing skills;
* Work collaboratively with the MDT, Adult Social Care, hospice and hospital staff, committing to ongoing learning and development.


Qualifications & Experience

• Clinical experience in palliative care, wound management, catheter management, cannulation, IVs and point‑of‑care testing techniques is advantageous.

• Demonstrated leadership ability and the ability to provide safe, effective treatments in a community setting.


Working Hours & Conditions

• You will need to be a car owner and driver.

• We operate a 7‑day service within core hours 8 am to 6 pm.
