Job Title: Rehabilitation Assistant - Guaranteed Hours/ Contract
Location: Taunton & Surrounding Areas
Pay: £27,040 per annum & paid mileage
Shift Times: 40 hours a week, alternate weekend work is a MUST.
* Morning: 7am – 3pm
* Evening: 2pm – 10pm
* Split: 7am – 2pm & 4pm – 10pm (with 2-hour break)
Key Responsibilities:
As a Rehabilitation Assistant, you will play a vital role in supporting individuals on their journey to regain independence following hospital discharge. Your focus will be on enabling recovery through person-centred, goal-driven care.
Your responsibilities will include:
* Delivering safe, consistent, and empathetic support aligned with each individual's rehabilitation plan.
* Working collaboratively with multidisciplinary teams, including discharge coordinators, occupational therapists, and community care staff, to support personalised recovery goals.
* Encouraging and empowering individuals to participate actively in their daily rehabilitation activities, promoting self-confidence and autonomy.
* Respecting each person’s dignity and preferences, and ensuring all care is provided with their full understanding and consent.
* Monitoring progress regularly and accurately, documenting achievements or concerns in line with care plans.
* Maintaining open, effective communication with professional teams and cluster groups to ensure a coordinated and responsive approach to care delivery.
