Overview
This pivotal role focuses on triaging and screening all referrals for young people with a learning disability aged 17½ to 18 into the adult learning disability team in South Essex. It is community based, requiring the post holder to visit homes, schools and day opportunities across Southend, Castlepoint and Rochford, Basildon, Brentwood, Wickford and Thurrock. A valid driving licence is essential.
Responsibilities
* Triage and screen referrals for young people with learning disability aged 17½‑18.
* Collaborate with mainstream services by developing pathways and providing education and training to promote reasonable adjustments.
* Support movement of service users between the wider MDT using the internal referral pathway.
* Lead weekly multidisciplinary team meetings, preparing information for internal referrals and complex case discussions.
* Provide comprehensive assessment and screening service; develop care plans reflecting service user needs and family/carers wishes; agree safe clinical outcomes.
* Collate accurate and relevant health and social care information to provide advice and guidance as the young person approaches their 18th birthday.
* Undertake comprehensive, evidence‑based clinical risk assessments and related care plans.
* Review and communicate risk assessments and management care plans in accordance with trust policy.
Qualifications & Experience
* Clinical skills and qualifications relevant to the delivery of care in the service area.
* Ability to comply with and promote the Code of Professional Conduct as stipulated by the relevant professional body.
* Holistic approach to assessment, treatment, review and management of packages of care, utilising resources effectively within the service area.
* Experience in working within a multi‑professional team.
Requirements
* DBS check – the cost is incurred by the employee and will be deducted from the first salary.
* Valid driving licence.
* Evidence of COVID vaccination in accordance with the Health and Social Care Act 2008 (Regulated Activities) (Amendment) (Coronavirus) Regulations 2021, unless exempt.
