Social Worker - Learning Disability within Adult in Leatherhead UK earning £38 Hourly Ongoing Full-Time

What you will do:
* Work as part of a multi-disciplinary team to deliver high-quality social work support to individuals with learning disabilities and autism
* Conduct assessments to identify the needs of service users and their families
* Coordinate care and support plans to ensure the needs of adults with learning disabilities are met efficiently and effectively
* Advocate for service users to uphold their rights and choices while adhering to relevant legislation and best practice guidelines
* Liaise with external agencies and professionals to provide comprehensive care planning and coordination
* Work collaboratively with families and carers to offer guidance, resources, and supportive services
