Duties
* Coordinate and schedule care plans in collaboration with clients, families, and care providers
* Carry out initial assessments and reviews for clients.
* Maintain accurate and up-to-date records of client information and care activities
* Liaise effectively with healthcare professionals, service users, and their families to ensure seamless communication
* Organise appointments, visits, and service delivery schedules to optimise client satisfaction.
* Monitor and review care plans regularly, making adjustments as necessary
* Provide administrative support including data entry, report preparation, and correspondence management
* Assist with onboarding new clients and staff members, ensuring smooth integration into the team
* Ensure compliance with organisational policies and relevant health and safety regulations
Qualifications
* Proven office experience is essential, demonstrating strong organisational and administrative skills
* Excellent communication skills, both written and verbal
* Compassionate attitude with the ability to handle sensitive situations professionally
* Strong organisational skills with the ability to prioritise tasks effectively
* Proficient in using office software such as Microsoft Office or equivalent programmes
* Ability to work independently as well as part of a multidisciplinary team
* A recognised qualification in health or social care is desirable but not mandatory
Requirements
* Full UK driving Licence, use of a car.
* Right to work in UK
* Previous experience in a similar role within healthcare is essential.
