About the role
As a Care Coordinator (Hourly Care), you will be the vital link between our clients and our incredible team of Healthcare Assistants. This is a dynamic, fast-paced role ideal for someone who thrives on relationship building, problem-solving, and delivering a consistently outstanding service. You will manage daily rotas, respond to last-minute changes, and ensure our clients receive the best care possible — with warmth, professionalism, and precision.
You will also take part in an
out-of-hours rota (6–9am/pm)
which is
paid separately to your salary
. This is on a
rota basis and not all the time
. On occasion, you may also be required to deliver care shifts in an emergency situation, though this is not a regular requirement.
What you will be doing
* Build strong relationships with clients and Healthcare Assistants
* Coordinate care rotas and manage short-notice changes
* Respond to client enquiries and set up tailored care packages
* Communicate with family members, local authorities, and healthcare professionals
* Keep all care plans and client records up to date
* Identify and respond to any safeguarding concerns
* Maintain compliance with care legislation and internal standards
* Support and motivate the hourly care team to deliver exceptional service
About you
You're a natural organiser and communicator who brings energy, compassion, and professionalism to every interaction. You may already have experience in care coordination, administration, or scheduling — but above all, you're passionate about helping others and making a meaningful difference.
You will have:
* Excellent communication and time management skills
* A supportive, proactive, and flexible attitude
* The ability to prioritise under pressure and stay calm in a busy, fast-paced environment
* Strong working knowledge of Microsoft Office
* A caring nature and commitment to high standards
* Experience working in a care or healthcare setting (preferred but not essential)
