Physical Health Lead Nurse (RGN)

We are looking for a confident Physical Health Lead Nurse (RGN) who can inspire, support and oversee the delivery of the very best care.
Cygnet Hospital Kidsgrove is our new facility for women in Staffordshire. The service now accepts referrals and offers a 20‑bed acute ward for women and an 11‑bed Highly Specialised Personality Disorder Rehabilitation ward.
There are two wards: Crocus Ward provides a safe and stabilising environment for women experiencing an acute episode of mental illness and requiring emergency admission; Burleigh Ward offers specialised support for women with a personality disorder and other complex needs.
Day‑to‑Day Responsibilities
* Assess, plan, develop, implement and evaluate programmes to promote health and well‑being, and prevent adverse effects on health.
* Oversee and evaluate individual care and treatment plans for patients with physical health conditions.
* Identify and manage care and treatment plans for patients at risk of developing physical health conditions.
* Provide regular monitoring and interventions for patients with long‑term conditions such as diabetes, asthma, hypertension, epilepsy, etc.
* Prioritise physical health problems and intervene appropriately in complex, urgent or emergency situations, including initiation of effective emergency care.
* Support patients to adopt health‑promotion strategies that encourage healthy living and self‑care principles.
* Provide information and advice on medication regimes, side effects and interactions.
* Assess and manage patients with wounds.
* Work closely with the visiting GP, oversee GP referrals, annual physical health check referrals, and referrals to specialist services.
* Coordinate with ward doctors for phlebotomy and ECG services to ensure accuracy and timeliness.
* Responsible for physical health induction and training for all staff.
* Implement the hospital‑wide smoking cessation programme.
* Oversee referrals to dietitians, opticians, dentists, hearing tests, chiropody and SALT.
* Regularly check physical health supplies/equipment and order as required.

Essential Qualifications
* At least two years’ experience as a Registered General Nurse.
* Ability to maintain documentation, undertake audits and demonstrate integrity in your role.
* Good understanding of latest nursing practices.
* Commitment to patient recovery in order to effectively monitor, manage and reduce risk.
* Compassion, resilience and a dedication to empower and support service‑user independence.
Desirable
* Ability to deputise in a Manager role.
* Interest in leadership, research and assessment.
