Our client, a well‑established social care organization based in South – Fawcett House, is currently seeking a dedicated and experienced Social Worker - Assessment Team to join their team on a contract basis. This role offers a competitive pay rate of £39 per hour umbrella and provides a hybrid working environment.
Candidate Responsibilities:
* Conduct assessments to determine the needs of individuals and families requiring social care support
* Develop and implement care plans tailored to the specific needs of each service user
* Work collaboratively with other professionals, such as healthcare providers and community services, to ensure comprehensive support
* Provide advice and guidance to service users and their families on available support services
* Maintain accurate and up‑to‑date case notes and records in accordance with data protection regulations
