What you will do:
– Safeguard and promote the wellbeing of children with disabilities through high-quality social work practice
– Conduct assessments aligned with the Social Services and Well-Being Act (Wales) 2014
– Develop and review care and support plans, ensuring they are monitored effectively
– Undertake Mental Capacity Assessments as per the Mental Capacity Act 2005
– Assess and mitigate risks promoting coproduction, independence, and choice
– Collaborate with various agencies and work within a multidisciplinary framework
– Maintain accurate and up-to-date electronic records in line with departmental policy
– Travel independently throughout the service area to meet the needs of service users
– Embrace remote working with one in-office day per week, providing a balance between flexibility and team cohesion
