Could you be Ty Hafan's new Paediatric Critical Care Liaison Nurse?

We are looking for an exceptional and experienced nurse to support us in delivering a test & learn project to strengthen care pathways between the Paediatric Critical Care Unit (PCCU) at Noah's Ark Hospital and Ty Hafan, and the wider paediatric palliative care team; ensuring families get timely access to hospice and palliative care support if needed. The role will support colleagues on the busy PCCU as they help families navigate the uncertainty and distress of having a very sick child.

Main duties of the job

If you have the passion and drive to make a difference to families facing into the worst times of their lives; experience of hospice care, specialist paediatric palliative care and of working on a busy intensive care or high dependency unit then you could be the person we are looking for.

The post is initially a 2-year fixed-term contract with continuation beyond this dependent on the impact of the role and securing funds. The role will mainly be based at the Noah's Ark Children's Hospital for Wales with regular interface with the Ty Hafan hospice team in Sully. There may also be a need to provide support in other hospital units across our catchment area. The role is full-time and primarily Monday to Friday, but a degree of flexibility to meet family needs will be required, along with some out-of-hours on-call duties.

Job responsibilities

Main Duties and Responsibilities:

Clinical:

Work with the PCCU team to identify children and young people who would benefit from palliative care and hospice input, facilitating timely and sensitive conversations about their goals of care, including parallel planning, with families and professionals involved as appropriate. This will involve:

- Attending ward rounds and relevant meetings as required

- Holistic needs assessment of patients on PCCU

- Liaising with the paediatric palliative care (PPC) team and/or children's hospice team as appropriate

- Being a resource for information and advice on available palliative care services to the PCCU and wider MDT teams

- Attending weekly PPC MDT meetings and participating in discussions about relevant patients, providing feedback to the PCCU team as necessary

- Supporting the PCCU and PPC teams in facilitating MDT meetings to discuss care where palliative care or parallel planning is essential

- Facilitating transfers to the hospice or other care settings for end-of-life care from PCCU or other acute settings, including arranging pre-transfer visits for families where appropriate

- Collaborating with hospital clinical teams, specialist nurses, and the hospice team to deliver a safe, effective, and holistic family-centered service

- Providing emotional, psychological, and spiritual support to families, respecting religious and cultural requirements, in partnership with the Ty Hafan Family Support Team and Health Board Teams

- Acting as a source of information and advice on palliative and hospice care when decisions about pathways of care are made for critically unwell children in acute settings across the area

- Supporting symptom management and palliative care of critically unwell children in collaboration with PPC specialists

- Assisting with discharge planning for children with palliative needs, maintaining communication with families and professionals post-discharge

Education, training, and service quality improvement:

- Collaborating with practice educators to provide education and training on palliative care across settings

- Maintaining your professional portfolio per NMC revalidation standards

- Engaging in ongoing learning, training, and research to stay current in palliative nursing

- Contributing to performance reviews, supervision, and quality improvement activities

- Supporting staff debriefs and ensuring pathways are in place for bereavement support

- Participating in policy and guideline development to enhance practice

Communication:

- Providing families with accessible written information about palliative and hospice services

- Developing collaborative relationships with children, families, and professionals, working across boundaries to support multiagency working at end of life

- Managing challenging situations with highly developed communication skills, including ethical dilemmas and grief

- Maintaining accurate, clear, and current records in line with policies and the NMC Code of Conduct

Other responsibilities include promoting equality, diversity, and inclusion, safeguarding, confidentiality, health and safety, and sustainability in all aspects of work.

The role is flexible, and duties may evolve; the post holder will undertake other duties as appropriate to their experience and training.
