WEEKEND SUPPORT WORKER ACCOMMODATION SERVICE, SUTTON-IN-ASHFIELD, 12 MONTH MATERNITY COVER, 24HRS, GRADE 2 AS - £24,398, FTE
AASA provides temporary supported accommodation for Homeless Adults who need assistance to find permanent accommodation. We work with people for up to six months who are over the age of 18 and require support as a result of being homeless. We have 17 flats at the service and 5 dispersed flats.
We are looking for someone to join our team who is passionate about promoting independence and empowering our service users to gain the skills necessary to live independently. AASA is a 24hr service, after a period of shadowing staff you will be lone working. You will work two 12 hr shifts Saturday and Sunday 08:00 20:00, giving a handover to our concierge service covering night shifts.
Duties & Responsibilities
To assess incoming referrals to the service and ensure that outcomes are logged. in line with contractual requirements
To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User.
To prepare agreed support plans with Service Users.
Sign post Tenants onto appropriate services for additional specialised support.
Assist service users with housing applications and move on options.
Assist service users with basic budgeting and life skills in order to support them to
move on to live independently.
To liaise appropriately with Key partners, stakeholders and service providers to
ensure appropriate support is offered to all Service Users.
To maintain the Health & Safety of the service, complete and record on-line forms.
