Gordon Street Surgery is seeking an enthusiastic and motivated Practice Nurse to join our friendly and supportive multidisciplinary team.

Main duties of the job
The successful candidate will be responsible for providing high-quality nursing care within a primary care setting, including:
* Chronic disease management clinics including diabetes, asthma, COPD and cardiovascular disease.
* Cervical screening.
* Immunisations and vaccinations.
* Wound care and dressings.
* Health promotion and lifestyle advice.
* NHS Health Checks.
* Long-term condition monitoring and reviews.
* Infection prevention and control activities.
* Supporting quality improvement and population health initiatives.
* Maintaining accurate and contemporaneous clinical records.
* Working collaboratively within the multidisciplinary team to provide patient-centred care.
Depending on experience and qualifications, opportunities may be available to develop specialist interests and lead clinical areas.

Job responsibilities
Job Purpose
To provide high-quality, patient-centred nursing care within a busy GP practice, supporting the prevention, diagnosis and management of health conditions while promoting health and wellbeing. The post holder will work collaboratively within the multidisciplinary team to deliver safe, effective and compassionate care.
Main Duties and Responsibilities
* Deliver a range of nursing services within scope of practice, including wound care, cervical screening, immunisations and vaccinations.
* Undertake health promotion activities and provide lifestyle advice to patients.
* Support the management and review of patients with long‑term conditions such as diabetes, asthma, COPD and hypertension.
* Perform clinical assessments and provide appropriate care, treatment and referrals.
* Maintain accurate and contemporaneous clinical records.

* Registered Nurse with current NMC registration.

* Cervical cytology qualification.
