Community Support Worker

An exciting opportunity has arisen to join the Mental Health Services for Older People Community Team in Whitby.

This team supports individuals and families within the community. You will work alongside a professional and caring multi‑disciplinary team to provide high quality care to those accessing the service.

Within this role you will develop a good knowledge of local services, support individuals in the community and local care residential homes, and communicate effectively with a wide range of individuals, adapting your communication style according to need.

You will need to work relatively independently and will receive regular supervision, support and professional development.

You will need to travel independently over a large geographical area and will require suitable transport.


Main duties of the job

* Providing a safe, patient-centred and effective clinical activity in the community setting.
* Monitoring mental and physical health, under the direction and supervision of registered staff.
* Delivery of non‑pharmacological interventions on an individual and group basis, focused on assisting the person to identify and achieve agreed goals.
* Supporting patients within care homes to monitor and review mental health needs and fostering positive working relationships and effective joint working between services.
* Providing timely physical interventions ensuring safe monitoring during medication changes and early detection of health concerns.
* Carrying out ECG or venepuncture as required – training will be offered.


Job responsibilities

This post is not open to trainees; therefore applicants must have completed the Diploma in Health and Social Care Level 2 or equivalent.

Applicants must also have Literacy and Numeracy Level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).

To work within the policies, protocols and clinical procedures of the Trust and to promote competent working practice within a supportive multi‑disciplinary team.

To liaise with Mental Health Nurses, Occupational Therapists and Physiotherapists within the team and carry out assigned duties as a member of the multi‑disciplinary mental health team.

To be prepared to work on own initiative as necessary.

Promote equality for all patients and understand their health needs.

Demonstrate an awareness of service user and carer perspectives on the provision of treatment and continuing care.

Ensure accurate record keeping in accordance with Trust policy.

Participate regularly in clinical supervision sessions.

Liaise with representatives from associated voluntary and statutory organisations (e.g. social care) and facilitate good communication processes.

Attend in‑service training and other study/refresher days to meet individual and service needs.

Participate in performance reviews.

Maintain safety, privacy and dignity of all patients in the delivery of patient‑centred care, recognising and respecting differences including spiritual and cultural beliefs.

Responsible for recognising signs of patient harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding of those who may be at risk and reporting all such concerns in accordance with Trust policy.


Person Specification


Qualifications

* Diploma in Health and Social Care Level 2 or equivalent.
* Care Certificate.
* Key skills in Literacy and Numeracy Level 2.
* Successful completion of Trust’s full HCA Framework within 6‑month probationary period.


Experience

* Previous experience as a Band 2/3 HCA or equivalent in a care environment.
* Previous experience of working with people with mental/social health needs or learning disabilities.


Other Requirements

* Ability to travel independently in accordance with Trust policies and service need.


Skills

* Effective verbal and written communication skills.
* Time management and ability to prioritise effectively.
* Competency in ECG and venepuncture.
