An exciting opportunity to join a dynamic multi-disciplinary team based in the emergency access areas of the Queen Elizabeth Hospital in King's Lynn, providing clinical expertise and specialising in admission avoidance services for the frail elderly population.
The post holder will provide clinical lead for an integrated, multi-professional admission avoidance team located within the Emergency Care & Assessment Areas. They will demonstrate strong interpersonal, communication, negotiation and delivery skills to manage a multi-stakeholder service.
Key responsibilities include:
* Planning, co-ordination and evaluation of physiotherapy, nursing and occupational therapy specialities within the emergency access setting.
* Providing specialist clinical knowledge in assessing frail, elderly patients with complex, multi-pathological problems.
* Responding promptly to calls and providing effective assessment within agreed standards to secure safe discharge to home or another suitable environment.
* Ensuring advanced combined assessment, diagnosis and treatment for patients with highly complex or chronic presentations.
* Training, educating and supporting Band 5 and Band 6 physiotherapists, nurses, occupational therapists, assistant practitioners, therapy assistants, students and other healthcare professionals.
* Sharing leadership with the Queen Elizabeth Hospital-employed counterpart to plan, develop, co-ordinate and evaluate the Rapid Assessment & Frailty Team.
* Organising and actively supporting the seven‑day working rota for the integrated team.
Person Specification
Qualifications
* Registration and qualification in a relevant professional discipline (e.g., nursing, physiotherapy, occupational therapy, paramedic sciences).
* Diploma or degree in a relevant profession.
* Evidence of postgraduate training in the relevant field.
* Evidence of a portfolio of CPD.
* Evidence of accredited or validated post‑registration education (e.g., leadership, management modules).
Experience
* Experience working with older people or those with complex needs.
* Experience of emergency or unplanned care.
* Previous experience of change management and service development.
* Experience of discharge planning and caseload management.
* Exposure to NHS priorities, processes and operational issues.
* Experience in implementing human resources processes for staff management and supervision.
* Experience working with an integrated service.
* Proof of successful change implementation.
* Experience working across professional boundaries and managing competing priorities.
* Previous senior-level experience.
* Experience of service redesign and user involvement.
* Experience in writing and implementing business cases.
