What you will do: Carry and manage a caseload of patients, collaborating with a Multi-Disciplinary Team (MDT) that includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dieticians, and more. Provide expert guidance to the team on social care roles and processes, including the Care Act. Engage in joint sessions, family meetings, and safeguarding meetings to ensure comprehensive patient care. Anticipate and address issues ahead of discharge planning to ensure smooth transitions for patients. Foster proactive discussions with patients and families regarding discharge options and community support. Signpost patients and carers to additional resources such as benefits advice and housing assistance. Problem-solve and coordinate with external agencies to address issues like housing, homelessness, or lack of public funds. Contribute to managing patient flow across the inpatient pathway, ensuring high-quality care. Participate in data collection and analysis to evaluate the impact and success of the role.
