About The Role
As part of our Reablement Team, you’ll play a vital role in transforming lives. You will provide short‑term, intensive support to individuals following hospital discharge or community referrals, helping them regain independence and stay in their own homes for as long as possible.
What You’ll Be Doing
* Deliver short‑term reablement support to help individuals regain daily living skills and independence.
* Support people in their own homes and communities to achieve independence.
* Undertake needs assessments to ensure care and support is tailored to each individual.
* Undertake Mental Capacity Assessments in line with legislation and best practice.
* Complete Continuing Healthcare (CHC) checklists to support eligibility decision‑making.
* Liaise with the Neighbourhood Team to ensure joined‑up, coordinated care and support.
* Focus on strengths and abilities – what people can do, not what they can’t.
* Work creatively to find solutions that improve lives.
* Collaborate closely with health professionals, including Occupational Therapists, Physiotherapists, social workers, and community services.
* Help individuals access community resources, activities, and support networks.
* Make a real, visible difference to people’s wellbeing every single day.
About You
* Passionate about making a difference.
* Confident communicator and team player.
* Creative, proactive, and solution‑focused.
* Experience in health or social care or holds a Level 2 qualification.
