Job summary
We are looking to recruit a salaried GP for between 4-8 sessions per week. We are open to negotiation around job plans which suit individual circumstancesand play to clinicians strengths.
An average surgery consists of 6 telephone slots and 8 face to face slots of 15mins duration. Thepractice operates a duty doctor clinic for same day urgent appointments. Youwill be expected to undertake a pro rata share of duty doctor clinics (whichare only ever a morning or an afternoon).
We also have an Emergency Care Practitioner and Physicians Associate who see patients for minor illness and injuries as well as conducting visits for housebound patients and care homes.
Wecontinue to develop and refine our clinical model and have a growingmulti-professional team including Clinical Pharmacists, Mental Health Practitioners, a First Contact Physio, Health and Wellbeing Coach and a range of other skilled practitioners.

Main duties of the job
Toprovide general practice consultations to patients, either face to face or viathe telephone, and where appropriate, physical examinations for the purposes ofidentifying or reducing the risk of disease or injury.
Touphold the principles of good medical practice as stated in the GMC statementof professional values and responsibilities.
Toarrange referrals for patients where appropriate.
Toensure that clear, accurate contemporaneous records are made of all patientencounters.
Carryingour home visits for patients
Dutydoctor cover
Checking& signing of prescriptions and dealing with associated queries
Preparingmedical reports for insurance companies, employers & statutory bodies forthe benefit of patients
Dealingwith paperwork & medical correspondence in a timely manner

Job responsibilities
1. To work with the Partnership in the delivery of PMS to the practice population by being responsible for the delivery of considerate and competent standards of medical assessments or advice to patients in order to deliver effective, safe and beneficial care according to the practice protocols, local guidelines and any other national service frameworks or guidance;
2. Offer opportunistic health advice patients on general health and in particular on the significance of diet, exercise, smoking, consumption of alcohol and misuse of drugs or solvents; and develop innovative approaches to delivery of health promotion;
3. To undertake surgery and telephone consultations, answer queries, visit patients at home, check and sign repeat prescriptions and deal with related paperwork and correspondence in a timely fashion;
4. To perform agreed medical services as the Practice determines to assist in achieving national and local targets for example the Quality and Outcomes Framework and such Directed, National and Local Enhanced Services as the Practice shall, from time to time, be contracted to participate in;
5. Offer consultations and where appropriate physical examination to identify and reduce risk to patient health and to motivate behavioural or circumstantial change to promote health;
6. To provide maternity medical services, child health development services and contraceptive services where qualified to do so;
7. To prescribe in accordance with the Practice prescribing formulary whenever clinically appropriate;
8. Undertake to effectively use all IT systems, and make every effort to adequately record, clearly and contemporaneously, all patient contacts on the clinical system (EMIS Web) using appropriate codes and templates;
9. Where appropriate to refer patients for specialist advice and treatment to other team members, to other providers of health services, partner organisations and secondary medical services, and facilitate access to these services;
10. To develop a continuing professional development plan, ensuring that you maintain a satisfactory level of training, in line with both the post holders personal needs and the needs of the Practice including participation in any clinical audit that my be required;
11. To work to support the development of the Practice initiating change and supporting others to work in new ways;
12. Attend and participate in all relevant practice meetings as required;
