We are seeking a motivated and patient-focused Clinical Pharmacist (Independent Prescriber) to join Penge PCN in Bromley. This role is ideal for individuals passionate about delivering safe, effective medicines management and contributing to the smooth running of day-to-day general practice activities.
Main duties of the job
As an onsite Clinical Pharmacist-IP you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.
You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvement of access to healthcare.

Job responsibilities
* As a clinical pharmacist-IP, you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's.
* Polypharmacy medication reviews and SMRs
* Face to Face medication reviews
* Help to utilise skill mix
* Ensure better access to healthcare
* Support practice staff with prescription and medication queries
* Medicine reconciliation and review post-discharge
* Conduct medical related audits and work on service improvement with the practice
* Provide expert advice on medications to other clinicians and patients
* Deliver clinical leadership on medicines optimisation and quality improvement
* Ensure appropriate and efficient use of medication
* Educate patients on medication and disease prevention
* An understanding of QOF, CQC and local incentive scheme workstream
* Create and follow clinical pharmacy programme according to practice policy
Person Specification
Experience
* Minimum 12 months experience working in general practice or primary care.
* Experience using EMIS
* Ability to travel to the practice site or reside within reasonable commuting distance.
* Excellent communication and interpersonal skills
Qualifications
* Registered Clinical Pharmacist with the General Pharmaceutical Council (GPhC).
* Completion of the CPPE Primary Care Pathway or evidence of exemption.
