Main duties
To undertake professional duties in patients’ homes and clinics delivering a high standard of nursing care and health promotion. The post holder will be responsible for the assessment, implementation and evaluation of the patient’s care programme. The holder is required to work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire NHS Foundation Trust. Work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the local population.
Job responsibilities
Clinical responsibilities
* Take clinical responsibility for patients within your allocated area of work.
* Continuously assess, plan, implement and evaluate treatment, using appropriate programmes/methods of care.
* Ensure a high delivery of quality care in accordance with Trust policies, procedures and guidelines.
* Participate in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives.
* Practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain your NMC registration.
Communication responsibilities

Contract
Permanent, full-time.
Salary
£32,073 to £39,043 a year (dependent on experience).
