<div><h3>Care Coordinator</h3><p><b>Department:</b> Management</p><p><b>Employment Type:</b> Full Time</p><p><b>Location:</b> Ripley</p><p><b>Reporting To:</b> Registered Manager</p><p><b>Compensation:</b>£28,700 / year</p><h3>Description</h3><p><b>Location:</b> Based at our Ripley branch – Old Post Office Building, Market Place, Ripley DE5 3FJ</p><p><b>Contract:</b> Full-time, permanent role</p><p><b>Salary:</b>£28,700</p><p><b>Please note:</b> We are unable to offer sponsorship for this role.</p><h3>Key Responsibilities</h3><p><b>Care Coordination & Scheduling</b></p><ul><li>Creating and managing person‑centred rotas on a weekly basis</li><li>Managing client runs and making changes to improve rota efficiency for team and clients</li><li>Solving rota issues as they arise, ensuring continuity of care</li><li>Managing staff holidays and sickness inputting into E‑days</li><li>On‑call responsibility on a rotational basis (one week every four) to handle out‑of‑hours contact until 10:30pm and ensure full call coverage as part of the management team</li><li>Liaising with professionals (e.g., Social Workers, GPs, District Nurses) and client families</li></ul><p><b>Office Administration</b></p><ul><li>Maintaining all aspects of office administration for carers, clients and all digital systems</li><li>First responder to office calls</li><li>Completing monthly carer visit note reviews</li><li>Inputting and updating staff data/records and availability into company systems</li><li>Responding promptly to office alerts, ensuring timely escalation if needed</li></ul><p><b>Quality and Compliance</b></p><ul><li>Managing invoicing notes and Purchase Orders in partnership with the Admin Exec and local authorities</li><li>Arranging care team introductions to clients when required</li><li>Preparing and submitting brokerage forms in collaboration with the Deputy and Registered Manager</li><li>Working in line with company policies and procedures to ensure the safety of clients</li></ul><p><b>Supporting Clients</b></p><ul><li>Covering care calls in office hours if needed</li><li>Supporting all stages of onboarding of new packages including client assessments</li><li>Ensuring a compassionate and professional telephone manner when talking with clients</li><li>Recognising client problems and working with colleagues to find solutions in the best interest of the client</li><li>Working in line with company policies and procedures to ensure the safety of clients</li></ul><p><b>Team support and wellbeing</b></p><ul><li>Providing guidance and support to the care team, addressing any concerns or challenges</li><li>Offering a supportive and caring presence and helping maintain morale and uphold Avanti’s values within the team</li><li>Leading by example in adhering to all company policies and procedures</li></ul><h3>Skills, Knowledge and Expertise</h3><ul><li>Good listener</li><li>Proactive</li><li>Supportive, caring and empathetic</li><li>Honest, kind and capable of setting professional boundaries</li><li>Responsive</li><li>Open to learning and driving own development</li><li>Able to manage time effectively</li><li>Organised and capable of managing multiple responsibilities</li><li>Committed to your work</li><li>Aspiring to progress in the sector</li><li>Professional</li><li>Flexible – this sector is unpredictable</li></ul><h3>Experience Needed</h3><p><b>Essential:</b></p><ul><li>A minimum of 2 years supporting and caring for people</li><li>A minimum of 1 year of experience in an office environment that involves producing and managing rotas on digital systems</li><li>Strong IT literacy in digital care planning systems</li><li>Strong experience in working with Microsoft Office or Google based software (or similar), email writing and managing an online diary</li><li>A Level 3 Diploma in Health and Social Care or working towards completion</li><li>A full UK driving licence and own vehicle; able to travel to clients’ homes as needed (travel can be expensed)</li><li>To live within reasonable distance of the branch</li></ul><h3>What we offer</h3><ul><li>Competitive salary package and opportunities for progression</li><li>Annual pay reviews to keep salaries in line with inflation</li><li>Ongoing training and professional development</li><li>A supportive, family‑run environment dedicated to the wellbeing of both clients and staff</li><li>The chance to be part of a growing team focused on delivering outstanding homecare</li><li>Pension – Employer contributions</li><li>Blue Light Card – Eligible to apply for the Blue Light Card and get access to discounts and rewards</li></ul></div>
