A position has become vacant in the Complex Discharge team at Barnet hospital for a Senior Complex Discharge Navigator. This is an opportunity to work with a dynamic specialist team at a senior level,
We are looking for an experienced, self-motivated, and passionate person to join our team. The post holder’s main responsibilities would be to provide oversight, management and support of complex discharges, working with the local Transfer of care hub and wider integrated services as part of the complex discharge team.
The post holder will also hold their own case load of the most complex cases, such cases would include patients with no recourse to public funds, homelessness, placements for patients with complex needs and patients who require the application of the Patient Choice Policy.
As the Senior complex Discharge navigator, the post holder will deputise for the Matron for Discharge and share managerial responsibility of a team of Discharge Coordinators and Patient flow coordinators. They will also be expected to provide ongoing live information pertaining to discharges.
Main duties of the job
Holding a case load of the most complex patients
Manage and support complex discharges to ensure that patients are discharged to the most appropriate setting in a timely manner
Deputise for the Matron for Discharge and ensure that all monitoring, reporting and escalating is provided both internally and externally to ensure all parties are aware of the position of the hospital. They will also be expected to provide live information pertaining to Non Criteria to reside patients and current delays.
To provide specialist discharge advice to multi professional teams, patients, and their advocates.
To escalate barriers to discharge both internally and externally
