In line with the philosophy of the clinical area, the post holder will be responsible for the assessment, planning, implementation, and evaluation of care for the service user, in conjunction with other staff. The post holder will provide a key leadership role, offering direction to the team, acting as a positive role model, and contributing to the professional development of junior staff while demonstrating Trust Values. The post holder will work closely with other professionals, including the wider Multi-Disciplinary Team (nursing, psychiatry, psychology, occupational therapy, and speech and language therapy) in assessing service users' needs, devising care plans, and evaluating outcomes that promote service user empowerment.
The role includes working as part of the Trust shift rota across a 24-hour shift pattern, including long day shifts and nights.
Main duties of the job
1. Provide leadership for the team, developing partnerships with inpatient services and primary, secondary, and tertiary healthcare providers.
2. Manage supervision and appraisal for junior staff, following HPFT supervision policies.
3. Act as a charge nurse for individuals with learning disabilities and associated mental health needs, formulating appropriate care plans.
4. Serve as a clinical expert, maintaining clinical updates, assessing service users thoroughly, planning care rigorously, and auditing interventions based on evidence and policies.
5. Consult with external agencies regarding clinical interventions, risk assessments, and management plans.
6. Coordinate all co-workers assigned to their service user group, ensuring they are fully informed about all aspects of the service user's care and MDT involvement.
7. Identify individual care needs, plan, implement, and evaluate care programs in collaboration with the community nursing team and MDT.
8. Provide clinical support and line management for nurses and healthcare assistants within the service.
9. Deliver person-centered nursing care that promotes the health and well-being of people with learning disabilities.
10. Maintain close liaison with all departments involved in service users' care, reporting on progress and effectiveness to the MDT.
11. Lead or participate in assessments within community and inpatient settings and prepare reports as required.
12. Ensure adherence to all relevant policies, legal requirements, and professional codes of conduct, maintaining current registration with the relevant professional body.
Essential requirements
* Relevant Professional Registration with Nursing and Midwifery Council (NMC).
* Mentorship Training and significant postgraduate experience within Learning Disability Service.
* Knowledge of physical, mental, and emotional health needs of people with learning disabilities.
* Experience working with people with learning disabilities or complex health needs.
* Understanding of legislation and policies related to learning disabilities.
* Experience maintaining high standards of recordkeeping compliant with legislation and policies.
* Physically fit to complete aggression management training and annual refreshers.
* Contribute to a culture of equality and diversity, recognizing and reporting undermining behavior.
