What you will do:- Assess, plan, implement, and evaluate specialist care for diabetes patients, ensuring individualised and evidence-based approaches.- Collaborate with multidisciplinary teams to develop and deliver holistic care plans tailored to patients' unique needs.- Educate and support patients and their families on diabetes management, promoting self-care and improved quality of life.- Contribute to the development of guidelines, policies, and strategies related to diabetes care within the team and wider organisation.- Maintain accurate and comprehensive patient records in compliance with professional and legal standards.
Requirements:To be considered for this role, you need a degree or equivalent in Nursing and be RGN qualified, with an active NMC registration for adults. Recent nursing experience in the UK is also essential, ensuring you are well-prepared to provide high-level care to diabetes patients.
