Senior Community Psychiatric Nurse - Community Rehabilitation
The Community Rehabilitation Service provides a flexible range of mental health interventions to help clients with complex mental health needs manage their mental health more effectively and remain in their own homes. The team works in close partnership with Social Care and housing providers to ensure each client has the necessary resources available to engage in their rehab journey. Across the Community Rehabilitation teams, the clinical team consists of Occupational Therapists, Psychologists, Nurses, and VCSE Workers.
We currently have exciting opportunities for enthusiastic and motivated Community Psychiatric Nurses to join our new West Kent Community Rehabilitation Service as the team's Senior Community Psychiatric Nurses. The successful applicants will care coordinate and provide rehabilitation nursing input to a defined caseload of individuals with complex mental health problems, deliver the therapeutic programme, and provide support and supervision to other members of the staff team. The team delivers comprehensive, individualised treatment and recovery plans, which include supporting clients to maintain their living environment and encouraging them to have meaningful roles and purpose within their local communities.
Individuals will be expected to ensure the team is able to provide input between the hours of 08:00 and 20:00 according to the needs of the service, including working some evenings, weekends, and public holidays.
Main Duties of the Job
The Senior Community Psychiatric Nurse is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver, and evaluate care, communicate findings, influence and drive change, and promote health and best practice, patient safety, and experience. The Psychiatric Nurse will be professionally accountable to the Executive Director of Nursing and operationally accountable to the Service Manager.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Essential Criteria
* RMN or RNLD
* Practice Assessor
* Completed Edward Jenner Programme
* Evidence of continuing professional development.
Experience
Essential Criteria
* Significant experience at Band 5.
* Evidence of post-qualifying continued learning.
Knowledge and Skills
Essential Criteria
* Significant knowledge of and ability to apply relevant legislative frameworks and policies relating to care e.g. Mental Capacity Act.
* Knowledge and understanding of the NMC Code of Conduct.
* A comprehensive understanding and ability to apply principles of risk assessment and risk management.
* To be competent in assessing, planning, implementing, and evaluating care.
* To pass medication and physical health competencies.
* Significant awareness of clinical governance and its relationship to clinical effectiveness, patient safety, and patient experience.
* Effective organisational/planning and decision-making skills.
* Good knowledge and practice of IT skills.
* Enhanced leadership skills e.g. taking charge, delegation of duties, supervising others.
* To participate in Quality Improvement, Audits, Research, etc.
