Responsibilities include:
* Conduct assessments to identify and prioritize needs
* Use SMART goal planning for needs-led holistic support
* Create an environment promoting development, learning, and independence skills
* Provide comprehensive support planning aligned with departmental policies
* Ensure clients are informed of their rights and responsibilities, encouraging involvement in service management
* Maintain accurate customer files and records of support provided in accordance with policies

Role Profile
Supporting Customers
Using Psychologically Informed and Trauma-Informed Care principles, support customers to develop independence through:
* Co-producing support and move-on plans with stakeholders
* Regular reviews and risk assessments
* Engaging customers in achieving outcomes and developing skills
* Supporting tenancy readiness and financial independence
* Monitoring healthcare needs and medication management
* Leading support initiatives and ensuring safety procedures are followed
Service Delivery
* Facilitate referrals and assess new customers
* Deliver housing management support, including income collection
* Maintain safe environments and report issues
* Carry out administrative duties
* Work flexible hours and travel as needed
* Participate in team activities and training
Person Specification
* Understanding of barriers faced by vulnerable groups
* Experience in structured support and risk management
* Empathy, flexibility, resilience, and decision-making skills
* Administrative and IT proficiency
* Knowledge of Trauma-Informed approaches and current benefit systems
