About Live Connected:
We provide quality care to adults with mental ill health, learning disabilities, dementia, and complex needs through supported living.
Key Duties and Responsibilities:
* Undertake daily living tasks as agreed with the service user, their family, and professionals.
* Work within the policies and procedures of Connected Living.
* Follow care and risk assessments specific to each service user's needs.
* Perform health and safety checks in line with policies.
* Work confidently alone following lone worker risk assessments.
* Ensure safe medication delivery and management.
* Respect service users' preferences and promote independence.
* Manage behaviors in the least restrictive way.
* Complete documentation and handovers accurately and promptly.
* Support clients' emotional and physical wellbeing as part of a team.
* Develop professional relationships with service users and multidisciplinary teams.
