This role provides an exciting opportunity for an experienced and motivated pharmacist to provide advanced clinical leadership within a well established, supportive multidisciplinary team in the Burgess Rd Surgery, Southampton whilst also supporting remotely Medina Healthcare on the Isle of Wight. The role is part of the Hampshire and Isle of Wight Healthcare NHS Foundation Trust Primary Care Pharmacy Team, working across Southampton and supporting practices on the Isle of Wight.
We are seeking an individual who is passionate about developing high quality primary care pharmacy services, driving meaningful improvement, and shaping innovative models of care across our network of practices. The role offers the chance to build a strong clinical portfolio while contributing to strategic service development.
You will work alongside senior pharmacists, GP leads and wider clinical teams, with structured mentorship and opportunities to further enhance specialist skills. Main duties of the job
The post holder will oversee the pharmacy workload for the PCN, focusing on governance and risk management, and will report issues and progress to the PCN clinical director. Please note that this role will not be based in a pharmacy.
Central to this vision is the need for resilient general practices that can navigate the challenges facing primary care while fostering innovation and the evolution of Integrated Neighbourhood working.
The patient group for this role includes those with complex medication needs or chronic health conditions requiring ongoing management and optimisation of their medication therapies.
This position will facilitate collaboration with general practitioners, nurses, and specialists in both primary and secondary care. The pharmacist will be a vital link in the multidisciplinary team, ensuring effective communication regarding patient medication histories and optimising treatment plans. Regular interactions with hospitals will enhance the information flow regarding patient medications, thereby improving continuity of care and patient safety.
The expected outcomes of this role include improved health results for patients through the safe and effective management of their medications, and enhanced efficiency within the primary care team. By addressing these issues, the pharmacist will help reduce hospital admissions, improve adherence to prescribed therapies, and enhance the overall quality of care in the community.

*assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team;

They will manage their own caseload of vulnerable patients at risk of hospital admission and harm due to poor medication management, as well as oversee the work of the network clinical pharmacist.
*The role involves running long-term condition clinics, where the post holder will be responsible for prescribing as an independent prescriber, particularly for conditions heavily reliant on medication, such as the optimisation of new antihypertensives or management of stable angina patients.
*The post holder will regularly review the ongoing necessity of each medication, assess monitoring requirements, and support patients in their medication adherence to ensure they derive optimal benefit from their prescriptions.
*Patients will be actively involved in decisions regarding their prescribed medications, with support offered to enhance adherence.
