Primary Vascular Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to perform on-site clinical assessments and determine appropriate patient treatment plans for residents of client facilities. This position reports to the Lead Nurse Practitioner.
The NP/PA will work in a mobile practice, primarily serving a long-term care setting.
Essential Functions
1. Perform on-site screening assessments to determine treatment plans for residents using comprehensive assessment forms.
2. Independently conduct history and physical examinations for complex acute, critical, and chronically ill perioperative patients.
3. Order and review diagnostic and therapeutic tests appropriate for the patient's age-specific needs.
4. Assist with care provision in accordance with facility, state, and federal regulations.
5. Monitor the effectiveness of interventions.
6. Maintain relationships with contacts in facilities.
7. Educate family members, clients, and prospective accounts about services and programs offered by Primary Vascular Care.
8. Assist with developing and maintaining physician relations, as assigned.
9. Complete weekly, monthly, and other reports promptly and comprehensively.
Qualifications
1. Bachelor's Degree required.
2. Degree from an accredited Nurse Practitioner or Physician Assistant program required.
3. Current NP License required.
4. BLS certification through the American Heart Association required.
5. ACLS certification preferred.
6. Current Infection Control Certification required.
7. Board Certified or Board Eligible required.
8. Physical exams, vaccines, and titers including PPD/TB, Hep B, MMR, Varicella required.
9. COVID and Flu vaccines preferred.
10. One year of NP experience preferred.
