Overview
City Health Care Partnership CIC are looking for highly motivated, enthusiastic individuals to join the developing team of practitioners based across Hull and East Riding.
Responsibilities
* The post holder will be responsible for the assessment, diagnosis and treatment of urgent same day need, and primary care patients. Presentations include undiagnosed undifferentiated illness, minor injuries, and exacerbation of long-term conditions. Consultations are performed via face to face, telephone, or a digital platform.
* Work collaboratively with partner organisations to ensure seamless and timely care for the best possible patient experience.
* Promote exceptional standards of patient care underpinned by evidence-based practice, audit, current guidelines and research.
* Act within professional boundaries, support the delivery of policies and procedures, and serve as a positive role model to junior staff while working flexibly to provide safe clinical expert care.
* Demonstrate effective leadership and be motivated, dedicated and enthusiastic. Work as part of a multi-disciplinary team and be exposed to a range of services and healthcare settings and locations.
* Work alongside a newly developed multi-professional clinical model and be supported by a network of clinicians to share expertise.
* Initiate clinical assessment including history-taking, clinical examination, diagnosis, treatment and referral, using advanced skills and expert knowledge for patients of all ages presenting with undifferentiated and undiagnosed conditions.
* Ensure the highest standard of clinical care is provided for patients across CHCP.
Working pattern
We provide services 24 hours a day, seven days a week, 365 days a year and therefore shift patterns will reflect this.
