Position Overview
Under the direction of the Manager of Admissions, the Hospice Flex Team Admission Registered Nurse (RN) facilitates speed to care by providing a range of preadmission and admission services. Collaborates with internal and community partners to resolve barriers to the Community Hospice admission. Provides education regarding Community Hospice services and philosophy. Collaborates with the manager, physicians, nurses, and case managers to ensure the best outcome for patients, families, and referral sources. Contributes to the smooth functioning of services, advocates for patient and family needs, coordinates care, and facilitates transfer of care to the appropriate team. Has an understanding of Medicare Conditions of Participation for Hospice, insurance payment processes, and State Regulations applicable to Hospice admission. Provides education and information to referral sources, physicians, patients, and their families. Represents the organization professionally, understanding the importance of the first impression.
Employees assigned to this role must be willing and able to flex their work location among various Hospitals, LTCs, ALFs, and home environments during each shift, as directed by Admissions Leadership.
Key Responsibilities
* Promote Community Hospice as the preferred provider of hospice care in the service area.
* Ability to effectively communicate the benefits of hospice care and address patient/family objections.
* Provide information about Community Hospice services and philosophy to patients, caregivers, physicians, LTC administrators, and the community.
* Implement Medicare Local Coverage Determination to assess hospice eligibility.
* Gather and communicate necessary information to/from physicians to determine hospice eligibility.
* Perform physical assessments, identify immediate needs including spiritual, functional, psychosocial, and financial aspects.
* Apply appropriate nursing interventions and document in electronic medical records.
* Facilitate admission to Community Hospice.
* Provide emotional support to patients and families with compassion and empathy.
* Maintain confidentiality in compliance with HIPAA.
* Assess patient risk and safety issues per policies.
* Develop initial care plans based on immediate needs, and obtain physician orders for medications, DME, and treatments.
* Demonstrate knowledge of medications for end-of-life care, including side effects and interactions.
* Coordinate care level changes and transfers based on Medicare Conditions of Participation.
* Adhere to professional ethical standards in service delivery.
* Engage in ongoing professional development regarding end-of-life care, Medicare Conditions, Local Coverage Determinations, State Regulations, and insurance benefits.
* Educate patients/families and staff on Advance Directives, including Health Care Surrogate and Proxy.
* Evaluate team members' adherence to policies and procedures, providing feedback to management.
* Mentor and educate new staff members.
* Build and maintain relationships with physicians, social workers, discharge planners, and community agencies to facilitate patient admissions and placements.
* Educate the medical community on hospice-appropriate referrals and benefits of early referrals.
* Represent Community Hospice professionally in all interactions.
* Follow organizational policies, procedures, and standards.
* Use resources responsibly and promote community awareness through participation in organizations.
* Continuously seek opportunities for professional growth.
Required Licenses/Certifications
* Current RN licensure in Florida.
* CPR certification.
* Valid Florida driver’s license with acceptable driving record; reliable transportation.
* Level II AHCA Background Screening.
* CHPN certification is a plus.
Education and Experience
* Combination of education and experience providing the necessary skills and knowledge.
* Preferred: Degree from a diploma or Bachelor’s nursing program.
* Minimum two years’ hospital or LTC experience, ICU or ER experience preferred.
* Reliable transportation to support work location flexibility.
