Overview
Job summary: This role is focused on supporting people in the community with a range of acute mental health needs, who may be in crisis, and their carers (including friends and relatives) and supporters, enabling and assisting them to meet daily health, social care and well-being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
We are looking for a mental health worker to work within North Somerset Intensive team. The position is offering full time hours. The team covers a 24/7 period and we work a shift pattern. Night provision is currently under review.
Responsibilities
* Undertake the role of mental health worker alongside a multidisciplinary team for an allocated caseload of service users, organising and participating in ongoing assessment, planning, delivery and review of care to meet identified physical, mental health, social care and well-being needs.
* Organise and participate in defined assessment methods/activities; develop and review appropriate care plans and identifying/organising other resources as appropriate.
* Provide defined interventions/activities as part of the care plan and complete relevant paper and electronic documentation in line with the care coordinator role.
* Plan and undertake defined interventions and treatments to enable individuals to meet and develop daily living skills, increase confidence and independence in line with personal recovery plans.
* Prepare for and deliver defined self-help groups; deliver therapeutic activities to improve coping skills (e.g., anger, anxiety, stress management, assertiveness, relaxation) and physical health (e.g., exercise, diet, nutrition).
* Facilitate activities to help individuals meet daily living goals (e.g., housing information, moving to new accommodation, accessing benefits, employment or education support).
* Provide emotional support, information and advice to maintain good physical health and to understand rights under the Mental Health Act.
* Promote continuity of care by maintaining regular contact with service users during inpatient episodes and work closely with other teams to facilitate early discharge.
* Work collaboratively with service users and their carers to understand and manage mental health needs in line with personal recovery plans and relapse prevention strategies.
* Organise and participate in the development of risk assessments and crisis management plans, rapid access plans, and report triggers to a registered practitioner with actions taken in crisis.
Qualifications and experience
* Essential: NVQ Level 3 in Mental Health or equivalent qualification, or demonstrated relevant experience; ability to work under pressure.
* Essential: Broad understanding of confidentiality; relevant experience in health or social care; ability to describe how supervision supports safe working practices.
* Skills: strong listening and verbal communication; ability to deliver therapeutic interventions within a prescribed framework; knowledge of policy and legislative frameworks (including rights under the MHA, CPA, Mental Capacity Act); willing to travel across the local area; hold a current driving license.
* Experience: previous experience in a mental health setting and in the community.
