We are excited to offer a rewarding opportunity within the Inverness district flow hub for a band 4 Health and Social care Co-ordinator whom form part of the Inverness Integrated team.
The HSCC play an integral role within the Integrated team through supporting the provision of person centred safe and effective delivery of care. The role involves managing referrals from professionals, patients, members of the public. Collating information from a variety of sources to support early multidisciplinary team decision making in order for delivery of care and service to that can be prioritised and utilised appropriately.
We are seeking someone who has excellent communication, interpersonal ski and organisational skills with a health and social care qualification or experience in the health and social care sector and demonstrate a good understanding of how health and social care is delivered. Competence with computer systems, attention to detail for updating of accurate record keeping.
Please be advised this role does not meet the eligibility criteria for a Skilled Worker Visaor a Heath and Care Worker Visa. NHS Highland is unable to provide candidates with a Certificate of Sponsorship (CoS).
