As an Outreach Recovery Worker, you will play a vital role in engaging with individuals in the community who are experiencing mental health or substance misuse issues. You will provide support, encouragement, and practical assistance to help clients achieve their recovery goals and improve their quality of life.
Responsibilities include, but are not limited to:
1. Conducting outreach activities to connect with individuals in need of support, including visiting clients in their homes or community settings.
2. Assessing clients' needs and developing individualized recovery plans in collaboration with them. Providing practical and emotional support to help clients work towards their recovery goals.
3. Acting as an advocate for clients, helping them navigate and access appropriate services, resources, and support networks.
4. Maintaining accurate records of client interactions, progress, and outcomes. Regularly reviewing and updating recovery plans to reflect clients' evolving needs.
5. Responding effectively to crisis situations, providing immediate support, and coordinating with other services to ensure client safety and well-being.
6. Collaborating with other professionals, including mental health services, substance misuse services, and social care teams, to ensure a coordinated approach to support.
7. Providing information and education to clients and their families about recovery strategies, coping mechanisms, and available resources.
8. Contributing to the development and implementation of community-based initiatives that promote recovery and well-being.
Requirements:
* Proven experience in a similar role or related fields such as mental health, substance misuse, or social care.
* Strong interpersonal and communication skills, with the ability to build rapport and collaborate with professionals.
* Understanding of recovery principles, mental health issues, and substance misuse; knowledge of local resources is advantageous.
* Empathetic, resilient, proactive, and committed to supporting individuals through their recovery journey.
* Enhanced DBS check dated within the last 12 months. If you are on the update service, you could be fast-tracked through our recruitment process.
* Willingness to complete an enhanced DBS if not currently held.
