Neighbourhoodnavigation (social prescribing) is an exciting NEW service for East Surrey enabling GPs, nurses and other health and care and social professionals to refer or recommend people into a range of local, non-clinical community-based services across the Joy platform.
The administrator role will form a key part of the new team delivering this new service. We would love to hear from you if you are a passionate, organised, and community-minded individual, with a strong background in providing admin support for a team or individuals.
The admin role will be a key role within the team, and you will be juggling a varied workload with often competing deadlines. You will work alongside the navigators, the Joy platform and other health professionals and respond to patient enquiries so excellent communication skills are a necessity.
Main duties of the job
The administrator role will have an interesting and varied workload. Working as the first point of contact for the new service and helping patients with any enquiries and paying attention to detail.
You will also be supporting the team of navigators and help with informing community groups and local support about Joy and the new service.
You will be working within the neighbourhood team to provide information or answering any queries that they may have.
You must be an effective team member but also be able to work on your own and use your initiative to resolve problems with minimal supervision.
Job responsibilities
The post holder will be supporting the new East Surrey Neighbourhood Navigation service (social prescribing) and the digital Joy platform and be responsible for providing effective and efficient administrative and clerical support to the team.
The service is being supported by Joy, a case management and marketplace platform that allows direct referrals from health professionals, and onward referrals into local community groups and clubs, allowing patients to access preventative care options that will lead to better, long term health outcomes for people.
The service will be aligned and integrated with the five East Surrey neighbourhoods and the local health and wellbeing networks and will work in partnership with key stakeholders in East Surrey including GP practices, Primary Care Networks (PCNs), Growing Health together, Local Authorities, Voluntary Community and social Enterprises (VCSE), Ageing well steering group, East Surrey Place, local community providers and Community link workers.
