Overview
This is a part time, permanent post as a Band 6 Practitioner for the City South LMHT, based at the Stonebridge Centre. We work with patients aged 18-65 with a variety of mental health diagnoses, including schizophrenia, bipolar, personality disorder, anxiety and depression. You will be community based, seeing patients both at the base and in their own homes, and you will work as part of a multi-disciplinary team in a fast paced environment that is exploring new and improved ways of working with our patients.
You will be part of a team with professionals including, but not limited to: Consultants, Pharmacists, Psychologists, CBT therapists, Mental health wellbeing practitioners, Nurses, Social workers, Occupational therapists, support workers, health improvement workers and peer support workers.
You will be managing a caseload of patients as a care coordinator, supporting the team on duty, managing patient and carer enquiries on the day. You will be conducting initial assessments of new referrals into the team and discussing patient care as part of a multi-disciplinary team.
Responsibilities

* Ensure accurate and timely documentation of patients’ care and report any incidents and safeguarding issues.
* Work with a patient-centred approach, involving families and carers in holistic care planning.
* Collaborate with the multi-disciplinary team to discuss and plan patient care.
* supervise junior staff and complete audits to ensure the service delivered is of high quality.
* Be responsible for the safe running of the service, including locking up the building as the team completes duties.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary after appointment. The DBS application costs are £26.40 (standard) or £54.40 (enhanced) and will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service, with an annual fee of £16 per year.
