New opportunity for a 16+ team based in North West England.
Responsibilities
* Manage a caseload of young people aged 16-18 within the Children in Care pathway.
* Undertake statutory visits, reviews, and care planning in line with legislation and local procedures.
* Work collaboratively with Personal Advisors, IROs, education providers, housing, health services, and other professionals to support the transition to independence.
* Ensure young people are prepared for leaving care by promoting their participation in pathway planning and decision-making.
* Carry out comprehensive assessments and prepare court reports as necessary.
* Ensure that safeguarding measures are in place and regularly reviewed.
* Maintain accurate and up-to-date case records and reports on the local authority's case management system.
Requirements
* Qualified Social Worker with a Social Work England registration.
* Minimum of 2 years post-qualified experience within children's social care, preferably in a CIC or Leaving Care Team.
* 3 years permanent experience.
* Strong understanding of the Children Act 1989, Care Planning Regulations, Leaving Care Guidance (2010), and related legislation.
* Experience working specifically with older adolescents (16-18) and supporting their transition into adulthood.
* Enhanced DBS on the Update Service or willingness to apply for one.
