Clinical Duties: Conduct advanced assessments, diagnose and manage patients with a wide range of conditions. Provide same-day/urgent care consultations within practices or at hub sites. Prescribe medications within scope of practice. Manage long-term conditions (e.g., diabetes, COPD, hypertension) in line with evidence-based practice. Request, interpret and act on diagnostic investigations such as blood tests, ECGs, X-rays, etc. Undertake home visits and care home rounds where appropriate. Leadership and Collaboration: Support the development of clinical pathways and integrated models of care. Contribute to service development initiatives aligned with PCN priorities (e.g., enhanced access, anticipatory care). Supervise and support junior clinical staff or trainees where needed. Documentation and Compliance: Maintain accurate and up-to-date clinical records in line with GMC/NMC standards. Participate in clinical audit, quality improvement projects, and appraisals. Adhere to safeguarding, infection control, and data protection policies.
