Are you an experienced Social Worker ready to thrive in a fast-paced, dynamic environment? We're seeking a dedicated and skilled Social Worker Officer to join our Adult Social Care Team - focusing on Front Door, Hospital Discharge, and Adults Assessment services.
Key Requirements
* Proven experience in a fast-paced adult social care setting, including:
* Front Door services
* Hospital discharge planning
* Adults Assessment Team work
* Familiarity with strengths-based practice models
* Strong working knowledge of safeguarding, assessment frameworks, and care planning
* Ability to manage a non-complex caseload and deliver high-quality outcomes under pressure
Responsibilities
* Conduct strengths-based assessments, reviews, and support planning to enhance independence and wellbeing
* Provide practical support to residents and carers, including access to aids, welfare benefits, and other resources
* Arrange short-term care and respite services to prevent carer breakdown
* Work closely with clients, carers, and communities to support informed decision-making and personalized care plans
* Liaise with health and care professionals across the Haringey partnership to gather insights and coordinate effective care
* Keep accurate case notes and reports, support legal documentation, and contribute to safeguarding processes
* Undertake care package reviews and contribute to multi-disciplinary team meetings for best outcomes
* Complete Continuing Health Care (CHC) checklists and identify candidates for full assessment
* Attend safeguarding strategy meetings where required
