You will play a key role in helping individuals sustain tenancies, develop life skills, and achieve positive outcomes as they transition into independent living.
Key Roles and Responsibilities:
1. Deliver high-quality, trauma-informed, and person-centred support tailored to the needs of each young person.
2. Provide flexible, intensive floating support within the community to promote independence and tenancy sustainment.
3. Manage a caseload independently, including planning your own diary and prioritising support effectively.
4. Support young people transitioning from supported accommodation into their own tenancies and local communities.
5. Utilise case management systems (e.g. SHARP) to review, monitor, and evaluate progress against outcomes.
